Uninsured or underinsured? 

Join Dr. Mike’s Smile Club!

Dr. Mike and his team are pleased to be able to offer memberships to our uninsured and underinsured patients*. 

Two plans will be offered:

PREPAID PREVENTION PLAN (PPP)

An annual flat fee (currently $475) entitles a patient to two preventative visits per year (cleaning, exam, necessary  xrays) and a 20% discount on any subsequent treatments

SMILE PLAN

An annual membership fee ($125) entitles patients to a 15% discount on all treatment provided during the one year membership.  All household members are covered by a single membership fee.

There are many benefits to joining our new Smile Club:

• - No annual maximums
• - No exclusions
• - No predeterminations
• - No denials

Indeed, Dr. Mike and his team will be able to provide the treatment you need and want without insurance companies dictating coverage.  If you already have insurance, compare what you’re paying and what is covered vs our Smile Club plans.

*(Smile club plans cannot be used in combination with any existing insurance plans a patient may have. Also, the Smile Club may not be used in addition to any other promotions we offer (eg. Senior Citizen Discounts, Care Credit…)
